Your notice must include all of the following, per § 512(c)(3)(A):

3. Identification of the material claimed to be infringing, with information reasonably sufficient to locate the material on our Services.
4. Contact information: your address, telephone number, and email.
5. A statement that you have a good-faith belief that the use is not authorised by the copyright owner, its agent, or the law.
6. A statement, under penalty of perjury, that the information in your notice is accurate and that you are authorised to act on behalf of the owner.

If you believe your material was removed by mistake or misidentification, you may submit a counter-notice under § 512(g) including:

1. Your signature.
2. Identification of the material removed and where it appeared.
3. A statement under penalty of perjury that you have a good-faith belief the material was removed by mistake.
4. Your name, address, telephone, and consent to jurisdiction of the federal court for the judicial district in which your address is located.

We terminate, in appropriate circumstances, the accounts of subscribers and account holders who are repeat infringers, in accordance with § 512(i).
